Abstract :

Type 2 Diabetes Mellitus (T2DM) is a chronic metabolic disorder characterized by insulin resistance, pancreatic β-cell dysfunction, and impaired glucose metabolism. While conventional treatments like metformin and insulin are widely utilized, there is increasing interest in herbal alternatives due to their potential efficacy, reduced side effects, and complementary mechanisms. Among promising botanicals, Gymnema sylvestre and Momordica charantia have shown significant antidiabetic activities in preclinical and clinical studies. Gymnema sylvestre contains gymnemic acids, saponins, and flavonoids that stimulate insulin secretion, regenerate β-cells, inhibit intestinal glucose absorption, and enhance glucose uptake in tissues. Momordica charantia provides charantin, polypeptide-p, and vicine, which mimic insulin action, regulate glucose metabolism, and offer antioxidant and anti-inflammatory effects. Clinical findings demonstrate reductions in fasting blood glucose, postprandial glucose, and HbA1c levels with these herbal extracts, although variability in phytochemical content and lack of standardized dosing remain challenges.
Comparative analysis of their pharmacological actions suggests overlapping and potentially synergistic effects when used in combination. However, limitations such as inconsistent extract quality, limited long-term safety data, and the need for large-scale clinical trials persist. Overall, Gymnema sylvestre and Momordica charantia hold considerable promise as adjunct therapies for diabetes management, working through mechanisms such as enhancing insulin secretion, improving glucose uptake, and combating oxidative stress. Future research should focus on phytochemical standardization, formulation advancements, and robust randomized controlled trials to fully establish their clinical efficacy and safety.
