PREHOSPITAL PARAMEDIC INTERVENTIONS AND THEIR IMPACT ON PATIENT SURVIVAL: A SYSTEMATIC REVIEW

Abstract :

Prehospital emergency care plays a pivotal role in determining patient survival outcomes before reaching definitive hospital treatment. This systematic review examines evidence on the impact of paramedic-led interventions—including airway management, cardiopulmonary resuscitation (CPR), defibrillation, hemorrhage control, and pharmacologic therapy—on survival rates across various emergency scenarios. Literature published between 2015 and 2025 was systematically searched in PubMed, Scopus, and Web of Science. A total of 48 studies met the inclusion criteria, encompassing trauma, cardiac arrest, stroke, and respiratory emergencies. Results demonstrate that early, protocol-driven paramedic interventions significantly improve survival to hospital admission and discharge. Advanced airway management and early defibrillation were the most strongly associated with favorable neurological outcomes. However, disparities exist across regions due to variations in training, resources, and prehospital systems. Strengthening paramedic competencies, integrating AI-supported triage, and enhancing real-time data exchange between field teams and hospitals are recommended to optimize prehospital survival outcomes.
