A COMPREHENSIVE REVIEW: ASSESSMENT OF THERAPEUTIC EFFECTIVENESS AND HEALTH RELATED QUALITY OF LIFE OUTCOMES OF ADJUVANT TERIPARATIDE THERAPY IN ORTHOPEDIC CARE

Abstract :
A serious public health concern, osteoporosis and accompanying fragility fractures significantly lower functional independence and health-related quality of life (HRQoL) in older populations. Conventional antiresorptive medications frequently fail to avoid mechanical hardware failures in complex orthopedic cases or speed up acute bone healing in tertiary care hospital settings. The therapeutic efficacy, radiological results, treatment sequencing strategies, and quality-of-life advantages of adjuvant teriparatide [recombinant human parathyroid hormone, $rhPTH(1-34)$] therapy for orthopedic patients are assessed in this review. With an emphasis on spinal, hip, and sacral insufficiency fractures, a thorough synthesis of multi-centric clinical trials, prospective randomized controlled trials, and real-world retrospective assessments was carried out. Standardized, established quality-of-life measurements, namely the Short Form-12 (SF-12) and the Quality of Life Questionnaire of the European Foundation for Osteoporosis (QUALEFFO-41), were used to critically evaluate patient-reported functional results and well-being. The combined evidence shows that teriparatide significantly increases systemic bone mineral density (BMD), encourages high-quality trabecular consolidation, and improves early radiographic bone repair. Perioperative teriparatide improves periprosthetic bone remodeling, guarantees pedicle screw stability, and dramatically lowers structural fixation failures such as implant subsidence and proximal junctional kyphosis (PJK). Additionally, switching from teriparatide to later antiresorptive regimens (such denosumab or alendronate) provides the best clinical route for long-term skeletal preservation.
Significant improvements in physical component states and acute mechanical pain management are revealed by standardized assessment scores from the SF-12 and QUALEFFO-41 tests, enabling quicker post-operative mobilization and a quicker return to independent living. In the end, incorporating teriparatide into tertiary orthopedic care protocols effectively bridges the gap between systemic biological bone recovery and mechanical hardware fixation, offering a highly effective, patient-centered pathway that successfully transitions people from immobility following a fracture to a state of restored health-related quality of life.
