282.EXAMINE THE FREQUENCY OF MYOCARDITIS AND ASSOCIATED MORTALITY IN PATIENTS WITH PARAPHENYLENE DIAMINE POISINING

Abstract :

Objective: To determine the frequency of myocarditis and associated mortality in patients presented with Paraphenylene Diamine (PPD) ingestion.
Study Design: Prospective/Observational
Place and Duration: Study was conducted at Services hospital Lahore for duration of nine months from November 2019 to July, 2020.
Materials and Methods: Total 110 adult patients of both genders presented with paraphenylene ingestion were enrolled in this study. Patients demographics including age, sex and reasons of ingestion of PPD were recorded after taking written consent from patients/attendants. Clinical presentation including sign and symptoms were recorded. The diagnosis of myocarditis was made solely on the basis of clinical signs/symptoms indicating myocardial damage, changes in electrocardiography, elevated cardiac biomarkers and trans-thoracic echocardiography abnormalities. Myocarditis-related mortality was reviewed. Data was analyzed by SPSS 24.0.
Results: 88 (80 percent) were women out of 110 patients, while 22(20 percent) were males. The patients’ mean age was 26.52±9.84 years. In 95.45 percent of cases, suicide was the most frequent cause of PPD ingestion. In 83 percent of cases, cervicofacial edoema was the most common symptom found. In 25 (22.73 percent) patients, myocarditis was found. 10(40 percent) of the 25 myocarditis patients died. The mortality rate overall was 27.27 percent.
Conclusion: Myocarditis has been found to be highly associated with the ingestion of praphenylene diamine with a high mortality rate.
