THE IMPACT OF MOTIVATION ON PARAMEDIC PERFORMANCE: A SYSTEMATIC REVIEW

Abstract :
Paramedics operate in high-stress environments where performance is critical to patient outcomes. Despite the demanding nature of the profession, the specific impact of motivation on paramedic performance remains inadequately synthesized in the literature. This systematic review aimed to synthesize the available evidence on the relationship between motivation and performance among paramedics and emergency medical technicians (EMTs). A systematic search was conducted following PRISMA guidelines across six electronic databases (PubMed, Scopus, CINAHL, PsycINFO, Web of Science, and EMBASE) from 2000 to 2024. Studies were included if they involved paramedics/EMTs and empirically examined the link between motivation and performance. Study quality was assessed using Joanna Briggs Institute (JBI) critical appraisal tools. A narrative synthesis was performed due to methodological heterogeneity. Of 2,347 identified records, 23 studies met the inclusion criteria. The findings consistently demonstrated that motivation is a multi-faceted determinant of performance. Intrinsic motivators—particularly autonomy, sense of purpose, and professional challenge—were primary drivers of enhanced clinical performance, better teamwork, and improved communication. Extrinsic factors, including adequate compensation, quality leadership, and career progression opportunities, were foundational for job satisfaction and retention. The evidence also revealed a strong inverse relationship between motivation and burnout, highlighting its role in career sustainability. Motivation is a critical, multi-dimensional determinant impacting all facets of paramedic performance, from clinical proficiency to personal well-being. Investing in strategies to enhance both intrinsic and extrinsic motivation is not merely a human resources concern but a fundamental prerequisite for ensuring high-quality pre-hospital emergency care and building resilient healthcare systems.
